Job Description 

CLINICAL PHARMACIST


Must have clinical/ hospital expereince

Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirement relative to tier age-specific needs, and to provide the care needed as described in the department's policies and procedures  Clinical Pharmacist shall provide clinical pharmacy services and drug information.

WORKING HOURS: 
Days (Mon-Fri,  7am- 3:30pm or  8am- 4:30pm)
 


Job Requirements 

Must have knowledge of the operation of equipment utilized in a hospital pharmacy, such as computers, printers, software, and documentation logs pertinent to pharmacy.

 
EDUCATION:
Graduate of an ACPE-accredited school of Pharmacy
Doctor of Pharmacy (Pharm.D) is preferred

Successful completion of a Hospital Pharmacy Residency Program or at least 3 years experience in institutional pharmacy with clinical experience

Active registration and in good standing with the Louisiana Board of Pharmacy.
