Job Description:
MDS Coordinator
The Minimum Data Set (MDS) Coordinator is responsible for coordinating, recording, updating, and monitoring all Minimum Data Set documentation required by state and other appropriate authorities.
Specific responsibilities include:
Manages the overall process and tracking of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for services provided within the Facility. Conducts concurrent MDS reviews to assure achievement of maximum allowable RUG categories. He/she will integrate information from nursing, dietary, social services, restorative, rehabilitation and physician services to ensure appropriate reimbursement.
· Responsible for updating and monitoring patient ICD-9 coding
· Regulates and maintains the MDS/PPS program and completion schedule.
· Develops and coordinates appropriate nursing care plan from MDS/RUG category.
· Will attend and contribute necessary information during Medicare meeting
· Will identify patients with significant changes after MDS review and refer appropriately appropriately
· Analyzes assessment data independently for complex patient problems and identifies early subtle changes that formulate RUG categories and effect patient care.
· Evaluates effectiveness of nursing care plans, and updates MDS/RA process within completion guidelines.
· Promptly respond to all emergency calls with appropriate emergency intervention.
· Provide care to residents including administration of treatments, medications, social & emotional support.
Requirements
We seek the following general qualifications:
- In depth knowledge of MDS required.
- Must have a valid nursing license - State of CT RN preferred
- 2 or more years experience preferred but will train the right individual.
- PPS experience preferred.
- Excellent technical and clinical assessment skills.
- Good communication and documentation skills.
- Previous experience computer systems and related software a plus.
- Solid ability to interact and build relationships with elders.
