                                                            Resident Care Manager

  

 

GENERAL PURPOSE To manage the nursing care of caseload while coordinating the efforts of the Interdisciplinary team to enhance the lives of the residents .

 

 

 Supervision

  1. Resident care manger will be responsible and accountable for managing the nursing care of his/her assigned residents including, but not limited to, supervising, directing, monitoring, and assisting in evaluating services, tasks and performance of direct care staff.

       

  2. Shall ensure maximum independence and self-direction for all residents.

       

  3. Shall coordinate the nursing functions and tasks for those residents with physicians and other health care providers.

       

4      Shall ensure the nursing plan and resident care plan are developed and documented, and that resident’s care needs are met.

       

 

5       Delegate authority;

       

       

      Nursing Care and Planning

           

       

       

1. Assist with new admissions, transfers and discharges.

           

           

2. Shall complete or delegate to other licensed personnel, documentation of the admission status of the resident within eight hours, including, but not limited to, skin condition, nutritional status, hydration status, mental status, vital signs, mobility, and ability to perform ADLs.

           

           

3. Shall develop or delegate development to other licensed personnel, a preliminary resident care plan within 24 hours of admission to ensure that the immediate needs of the resident are met. Shall ensure that staff, providing care for the resident, shall have access to, be familiar with and follow through with this plan.

           

           

4. Shall develop a restorative plan based on a review of the residents needs and delivered in accordance with the resident care plan.

 

C) Documentation

        

    1. Review and complete resident records and other related documentation in a timely manner. Sign, date and perform all charting and record keeping in accordance with established policies and procedures.

       

    2. Document all pertinent information in the interdisciplinary notes and on the 24 hour DNS report form.

       

    3. Ensure daily documentation of skilled care residents.

       

    4. Ensure completion of daily, weekly or quarterly documentation, as required, which reflects current resident care and condition.

       

    5.  Complete a summary that recaps the resident’s stay, status, nursing review upon discharge, and a post discharge plan of care.

        

 

D) Other Job Functions

        

    1. Assist in coordinating resident and family support groups.
    2. Develop and implement methods for determining facility quality.
    3. Ensure that work areas are clean, safe and orderly
    4. Ensure facility fire safety, hazardous material, and infection control practices are being followed.

       

       

 

 

           

 


Job Requirements 

MINIMUM REQUIREMENTS

       

    •  Must have current state R.N License in Washington

       

  • Graduate from an accredited professional nursing program

       

  • Ability to read, write and follow oral and written directions at a level necessary to accomplish this job.
  • Must be capable of maintaining regular attendance

       

  • Must be capable of performing the essential functions of this job, with or without reasonable accommodations.

      PHYSICAL REQUIREMENTS

      Visual and auditory acuity within normal limits, fine hand coordination, frequent standing and walking. Bending and kneeling, reaching, grasping, pushing and pulling wheelchairs and other equipment. Lifting residents (70-290lbs) with assistance as indicated, move residents in bed (40 lbs), up in bed (60lbs), chair to stand (60lbs). Ability to distinguish smells and temperatures. Ability to read, write, and communicate with residents, personnel, and visitors. Ability to remain calm under stress.
