Indiana Heart Institute
is seeking RNs to fill full-time data abstractor position(s). Please be aware that this position DOES NOT involve direct patient care or contact.

This position DOES require a Saturday or Sunday rotation shift every 3-5 weeks (differential is offered-RN choice of Saturday or Sunday.)

The Clinical Resource Coordinator is a skilled professional nurse (healthcare provider) who is responsible for collecting, screening and analyzing data that profiles the appropriateness, utilization management, quality and outcome of patient care provided by the medical staff. This individual must maintain professional confidentiality as privileged information will be retrieved, studied and recorded.

This individual is responsible for knowledge of specific data requirements as defined by the Society of Thoracic Surgery (STS), American College of Cardiology (ACC), Peripheral Vascular Registry, Ascension Health and Joint Commission Accreditation of Hospital Organization (JCAHO) which will be entered into the Cardiac Registry(HeartBase). HeartBase is the primary data support source for the Quality Improvement Program at St. Vincent Heart Center of Indiana. This individual performs concurrent and retrospective reviews/monitors which evaluates the quality of patient care. This individual is also responsible for thoroughly analyzing the medical records in order to accurately code and abstract medical data. This information is submitted for financial reimbursement as required for the Uniform Bill and the DRG/Prospective Payment System. This individual will possess excellent clinical judgment in the area of CV related care and outcomes management. This individual will be expected to perform in a leadership role within the Quality Improvement Process for the CV Serviceline.

Supporting the philosophy and core values of the Indiana Heart Institute and the affiliate hospital is an integral part of this position.

MAJOR JOB DUTIES AND RESPONSIBILITIES:
• Interacts with the medical staff and their appointees to obtain and provide pertinent information for the CV Quality Improvement Committee.
• Performs quality and clinical review and evaluation on a case by case basis in a timely manner.
• Provides support and coordination for assigned QI Committees within the hospital structure.
• Monitors/collects data for established clinical indicators and utilization indicators.
• Monitors assigned divisions’ compliance with JCAHO standards.
• Performs data collection (concurrently and retrospectively) of specific data as defined by the Society of Thoracic Surgery (STS), American College of Cardiology (ACC), Peripheral Vascular Registry, Centers for Medicare and Medicaid Services (CMS), all JCAHO variables and required internal data elements that support PI initiatives at the hospital.
• Monitors and appropriately queries physicians for quality documentation to support ICD-9-CM coding and DRG assignment.
• Performs peer review functions and provides support to hospital Medical Affairs Department
• Participates in data collection for research and ad-hoc quality improvement projects as requested.
• Assists in development, designs of methodology and the data collection process for HeartBase. Monitors documentation within HeartBase and acts as a liaison with all users of HeartBase.
• Maintains current knowledge of sources of outcome data and quality tracking.
• Interacts with Case Management/Care Coordinator’s to accurately depict the hospital stay (admission order, queries, etc) The following major functions describe the essential duties of positions in this job classification. • Assist with clinical pathway development, education.
• Utilizes appropriate communication skills with patients and their significant others if necessary.
• Utilizes appropriate problem solving and critical thinking skills in working with the CV population.

Team Responsibilities:
• Engages in a team building efforts and is willing to voluntarily wok in other positions within the IHI organizational structure as the needs of the business dictate.
• Participate in the team decisions regarding staffing, quality monitoring, and continuous improvement of the HeartBase/coding process.
• Refers HB/coding questions to Process Leader and/or physician to ensure consistent data collection.
• Assures data collection is timely.
• Communicate effectively with co-workers and physicians ensuring a good working relationship.
• Participate in data clean-up of HeartBase data entry.
• Keeps abreast of clinical knowledge and coding, DRG changes.
• Abstracts clinical data required by STS, ACC, CMS, CIHS and Vascular in a timely manner as noted by feedback from individuals receiving reports and administrative observation.
• Establish and maintains an accurate and compete longitudinal database as observed by supervisor.
• Maintains less than a 2-5% error rate which is established on the total number of charts abstracted per year and the number of errors identified.
• Maintains knowledge of data definitions and data entry guidelines.
• Uses available resources, I.E., data dictionary, Coding Clinics, ICD-9-CM Code Books to abstract the patient’s clinical picture and performs accurate data entry as observed by administrative observation.
Requirements 1. Graduate of an accredited school of nursing, hold a diploma of nursing, or certificate of nursing.

2. Must hold a valid RN license within the state of Indiana.

3. Must have strong experience using Windows.

4. Must have average skill level for Microsoft Word and Excel.

5. Cardiac Experience is greatly preferred, but not required.

6. If a candidate DOES NOT have cardiac experience, he/she MUST have a strong independent learning ability. Education outside of normal work hours WILL BE expected.

7. Candidates MUST be flexible and team players.

8. This position DOES require rotating weekend work. Current RNs rotate one Saturday every 4-5 weeks.

9. Travel to various cardiac facilities MAY be required from time to time around the city of Indianapolis, or surrounding counties, may be required,- a high estimate of travel required is 25%
