General Dentists and Endodontist:

Shall be a graduate of an ADA-accredited dental school with either a Doctor of Dental Surgery (DDS) or Doctor of Dental Medicine (DMD) degree.  Shall possess a current, active, valid, and unrestricted license in at least one of the 50 states, and/or the District of Columbia, the Commonwealth of Puerto Rico, or territories of the United States.



General Dentists.  Shall have been employed for at least one (1) year on a full time basis in the practice of dentistry, or be a recent (1-year) graduate of an accredited, post-graduate dentistry training program.



Endodontists.  Shall have a certificate of training in endodontics from an ADA-accredited residency training program and be board eligible or board certified in endodontics.  Endodontists shall be capable of performing the full range of dental services commensurate with that of a general dentist in addition to the services of their specialty.
