Description
Shall be a graduate of an ADA-accredited dental school with either a Doctor of Dental Surgery (DDS) or Doctor of Dental Medicine(DMD) degree. Shall possess a current, active, valid and unrestricted license in a least one of the 50 states, and /or the District of Columbia, the Commonwealth of Puerto Rico, or territories of the United States.
Endodonists. Shall have a certificate of training in endodontics from an ADA-accredited residency training program and be board eligible or board certified in endodontics. Endodontists shall be capable of performing the full range of dental services commensurate with that of a general dentist in addition to the services of their specialty.
