Description
MDS Coordinator
Parthenon Healthcare of Blountstown is a 150 bed skilled-nursing facility located in Blountstown, FL. We also received a Deficiency Free Survey for 2008. Our facility has a Special Care Unit that excels in caring for residents with cognitive disorders such as Alzheimers’ and Parkinson's Disease. It has its own outside courtyard where residents are free to walk, to garden, participate in recreational activities, or just relax. We also have a Hospice unit with dedicated staff to meet specific needs of residents.
We are currently seeking an MDS Coordinator. Duties include but not limited to:
- Conducts and coordinates the development and completion of resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment.
- Develop, implement, and maintain an ongoing quality assurance program for the resident assessment/care plans.
- Coordinate the interviewing of each resident for the resident’s assessment.
- Work with the Interdisciplinary Care Plan Team in developing a comprehensive resident assessment and care plan for each resident.
- Participate in facility surveys (inspections) made by authorized government agencies.
- Meet with residents, and/or family members as necessary. Report problem areas to department directors.
- Work with team members to ensure discharge planning goals and objectives are developed and discussed at the interdisciplinary team meetings.

Requirements
MDS Coordinator

Must be a Registered Nurse (RN) in good standing and currently licensed in the State of Florida.

Must possess, as a minimum, two (2) years, experience in hospital, nursing care facility, or other related health care facility

Must have training in rehabilitative and restorative nursing practices.

Must have knowledge of the Minimum Data Set Tool.

Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.

Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
