INSPIRIS is a unique care and care management company focused on improving the health care and quality of life for the medically complex, chronically ill, frail and elderly patient. INSPIRIS offers nurse practitioner led programs for Skilled Nursing Facilities, Assisted Living or other congregate housing and in individual residences. These programs can be as short as a single visit for clinical and HCC Risk Scoring assessment, short term for management of post-hospital discharged members to long term for those members not accessing the current PCP network.

We have immediate opportunities for Nurse Practitioners to conduct Home Assessments in the Dallas/Fort Worth areas.

Our Home Assessment program offers the opportunity to complete a full history and physical assessment for patients in the comfort of their own home during a single visit.  This position allows you to earn additional income by completing these assessments on a per diem basis.  The Nurse Practitioner is provided the necessary equipment and medical supplies needed to perform the assessments, and is required to submit documentation at the completion of the assessment.   This position requires travel to patients' homes.  Excellent compensation.
