RN- Community Health Nurse

RN- Community Health Nurses

• Full Time, Part Time, Per Diem and On-Call positions available. 
• Any combination of weeknights, weekends or overnight schedules.
• Minimum 2 years clinical experience. 

Summary
  

• Administer skilled nursing care to patients requiring professional nursing service. 
• Educate the patient and family on care. 
• Supervise other members of the clinical team. 
• Perform these services in accordance with the physician’s orders and under the direction and supervision of the Director of Clinical Services/Clinical Team Manager/Team Leader.

Responsibilities include

• Providing and coordinating quality patient care
• Maintaining a number of cases
• Maintaining productivity goals with consideration of case mix
• Accurate, timely documentation.  Responsible for the overall plan of care and the outcome of that care, including care delivered by other nursing staff (LPN, HHA) who must be directed and supervised by the CHN. 

Qualifications
  

• A registered professional nurse with a baccalaureate degree in a health or human services field

 or 

• Registered nurse with (2) two years of experience in home care.
