job description

  • Coordinate population-based activities for all chronic diseases through guideline development and dissemination, assisting with implementing disease specific intervention strategies and actively participating in implementation of improved delivery systems.
  • Assist members in developing strong self management skills by actively engaging them in the disease management program.
  • Use exceptional critical thinking and communications skills in facilitating changes in behavior.
  • Analyze data from a variety of sources to help identify patients, determine needs and evaluate trends.
  • Work with computer system and software to access and maintain disease management information.

job Requirements

  • Requires 5 years of related progressive experience.
  • Requires Bachelor Degree in nursing, biology, related field, or the combination of education and experience that would enable performance of the full scope of the position.
  • Requires a current unrestricted registered nurse certification with the Colorado State Board of Nursing or hold other equivalent professional licensure or certification related to chronic diseases (eg. diabetes, coronary heart disease, asthma). Must have a valid Colorado driver’s license with proof of vehicle insurance and reliable transportation.
