Our Home Health Agency is currently accepting resumes for the position of RN Case Manager. The RN provides direct care to clients including assesments, treatments and interventions. Communicating concerns to clients, Dr., family and educating patients how they may achieve a healthier quality of life . Our Home Health Agency is in the preliminary stages of licensing for a Hospice program, and Case Managers aid in the call responsibilities for Hospice patients as well as the care of pre-hospice/palliative patients. Call schedule is roughly every 5 weeks.
Requirements
Required:
-Valid RN license
-Minimum of 1 year of med-surg or home health experience
-Reliable transportation
Preferred:
-2+ years of home health experience/ community health experience
