We are currently seeking a Nurse Practitioner in the Heart Center!
The Nurse Practitioner:
- facilitates the often complex referral process for adult patient with Congenital heart Disease Program.
- reviews new patient referral requests to facilitate timely access to care.
- Assists with insurance/pre-certification issues when needed.
- Coordinates outpatient tests with in both Pediatric Cardiology and Adult Cardiology and specialties outside the service line.
- Receives and triages all ACHD patient phone calls and follow up.
- Monitors prescription drug management.
- Assist with post discharge and follow up cares
- Provides Patient/family education and work with CT surgery and other procedure areas to arrange for invasive procedures.
