NJ MENTOR - Founded in 1980, The MENTOR Network is a national network of local human services providers offering an array of quality, community-based services to adults and children with developmental disabilities or acquired brain injury; to children and adolescents with emotional, behavioral and medically complex challenges; and to elders in need of home care.
***Overnight Part Time Positions***
Tthe Direct care staff assist individuals in accordance with the Service Plan for the purpose of direct care services, behavioral redirection, training, and support to address the functional/daily or community living skills needs of children and adolescents with emotional or behavioral challenges. These duties may be performed in one or more setting.
Responsibilities:
· Establish a therapeutic relationship with the individual receiving services to ensure effective guidance, support and service delivery.
· Attend monthly treatment meeting(s).
· Provide training, assistance and/or supervision to individuals in the areas of daily living, personal care, financial management, community and social integration and/or job skills development.
· Provide pre-vocational skills training for a minimum of 5 hours weekly.
· Provide Lifeskills training for a minimum of 3 hours weekly.
· Provide support to the individual to ensure objectives are met in accordance with the Service Plan.
· May be required to coordinate, organize and/or assist with household activities such as light housekeeping and meal preparation.
· May provide transportation for individuals receiving services to planned and/or necessary activities and appointments.
· Maintain current progress and service notes and any other appropriate documentation in accordance with The MENTOR Network policy, program and State statutes.
· Complete and submit all required documentation to direct supervisor in accordance with program deadlines.
· Provide direct supervisor with verbal feedback and data relevant to the individuals’ day to day services, Service Plan or administrative policy.
· Actively attend and participate in multi-disciplinary team meetings as required.
· Attend training and other meetings as required.
· Provide emergency coverage and/or care for individuals receiving services.
Maintain confidentiality, respect human rights and practice universal precautions in accordance with The MENTOR Network policy
Requirements
· Baccalaureate degree or high school diploma and 3-5 years of experience providing direct care to emotionally disturbed youth in a behavioral health agency or institutional setting.
· At least 21 years of age and able to read and write.
· Ability to apply common sense understanding to carry out instructions furnished in written or oral form.
· Ability to deal with routine problems involving several concrete variables in standardized situations.
· Must possess current driver's license in good standing and may require registration and insurance on personally owned vehicles used to transport.
· Successful clearance of three (3) reference checks, local criminal background checks and successful fingerprint clearance and any further checks required by state regulations.
