Job Description

Virginia Beach Health Care Center is seeking a full time Social Worker for a 240-bed long term care and rehabilitation center in Virginia Beach, VA.  The social worker will coordinate all dischrge planning initiatives for assigned caseload, spearheading from the point of admission, through discharge, to the point of relocation.  He/She will provide comprehensive medically related social services to all assigned patients.  The qualified candidate will operate within the framework of MFA's standard of best practice as well as state and federal regulations.

 

The qualified candidate will have a minimum of a Bachelor's degree in social work or a Bachelor's degree in a Human Services field including, but not limited to  Sociology, Special Education, Rehabilitation Counseling, and Psychology.  A minimum of one year, two years preferred, of supervised Social Work experience in a healthcare setting working directly with individuals.
