Client Care Coordinator for Private Duty Non Medical Home Care Agency -
Looking for that special someone with good home care experience at assessing clients needs for home care. You will be assessing clients, working with families trying to help their loved ones decide on getting help at home, developing a plan of service, and overseeing care for all clients. Should have worked extensively with dementia clients. Enjoy the process of selling clients on why your firm can help them with their needs. You will be responsible for all clients needs, and concerns, problems and joys. Be a problem solver.
