Description

POSITION:                Case Manager II

LOCATION:              Safe House – Yuma, AZ

HOURS:                    Full Time, Non-Exempt, All Shifts

 

 

PRIMARY FUNCTION

 

Provide referrals, resources and support to residents of a domestic crisis shelter, transitional living program and the community. Meets the needs related to day-to-day living such as housing, transportation, employment, education, budgeting, community involvement and family support.  May perform other duties as assigned.

 

ESSENTIAL FUNCTIONS

 

  • Promote the safety and confidentiality of participants and shelter
  • Provide protection, food and other basic needs with dignity and respect to program participants
  • Utilize the “Empowerment Model" of behavior
  • Provide information, resources and referrals through a supportive and nurturing environment.
  • Participate in the rotation of answering the crisis line and assist with participant intake
  • Collaborate and network with other agencies to provide services that enhance community awareness on DV and promote the Safe House Program
  • Reduce isolation by reconnecting participants with natural support systems
  • Provide individual and systems advocacy
  • Aid participants in expanding their choices and achieving violence free lives through setting goals
  • Participate in weekly staff meetings and required trainings
  • Required to be on-call basis depending on program needs
  • Facilitate individual and/or group sessions in a culturally sensitive manner
  • Maintain confidentiality and the safety of shelter and participants
  • Adhere to licensure regulations, contract compliances and program requirements
  • Complete and maintain accurate notes, logs and documentation
  • Aid in the completion of reports (weekly, monthly, quarterly)
  • May drive agency or personal vehicle on company business
  • May perform other duties as assigned

 

Requirements

MINIMUM QUALIFICATIONS

 

  • BA in human services, or related field.
  • 2 years of related experience
  • Obtain State of Arizona Fingerprint Clearance Card
  • Bilingual English / Spanish, verbal and written
  • Valid Arizona Driver License, proof of insurance and Arizona Motor Vehicle Division 39 month Motor Vehicle Report
  • Ability to obtain health care certificates including CPR, First Aid, Food Handlers Certificate, and TB clearance
  • Proven strong communication skills – oral and written are clear, concise, and in an organized fashion using appropriate style, grammar and tone
  • Ability to effectively facilitate individual and group sessions
  • Ability to develop and maintain effective working relationships with other staff, officials, community organizations, government agencies and the general public
  • Ability to stoop, climb, bend, and reach
  • Required to lift 35lbs. 15%

 

 

DESIRED QUALIFICATIONS

 

  • Additional years of directly related experience
  • Previous experience working in a residential setting
  • Previous experience in group facilitating
