Description Broomall Rehabilitation and Nursing Center a 298 bed Skilled Nursing/Sub-acute Rehabilitation and Long Term Care Facility located in the beautiful Western Suburbs of Philadelphia; one mile form the Blue Route (I-476), is searching for a part time, (3 days per week) self-starting, dynamic, goal oriented Social Worker with leadership experience. We are a EOE/AA facility.

Requirements This successful candidate will be flexible and possess an ability to prioritize. Knowledge of MDS documentation, care planning, discharge planning and complaint resolution a must. Qualified candidates will possess at a minimum a BSW (MSW preferred) and one to two years of experience as a Social Worker in a long-term care setting is preferred. Experience with other aspects relative to long-term care clientele, such as dementia and short-term rehabilitation will also be weighted favorably.
