POSITION SUMMARY: Responsible for the delivery of professional physical therapy services, which will address the physical, psycho-social, spiritual, cultural, developmental, and age-appropriate needs of each individual patient/family. Assists patients in achieving optimum levels of functioning via evaluation, designing a treatment plan, and implementing and coordinating care with team members. Under the direction of the physician, the physical therapist will educate and treat patients with discomfort and pain, dysfunctions, limitations, and deformities. Physical therapists will utilize physical means of sound, light, electricity, water, heat, cold, exercise, and manual interventions. Documents plan of care, rehabilitative potential, and discharge plan.
ESSENTIAL FUNCTIONS
1. Performs patient assessment; applies diagnostic and prognostic muscle, nerve, joint, and functional ability tests
2. Develops plan of care; including discharge plan based on the patient’s potential.
3. Implements care and maintains up-to-date treatment records
4. Assess patient/family learning needs
5. Educates patient, family, and other staff members regarding patient care, exercise, equipment, devices and disease process
6. Evaluates plan of care
7. Develops a plan of care for physical therapist assistants and/or physical therapy aides to follow and overseas their facilitation of plan
8. Participates in interdisciplinary, Rehab, and Skilled meetings, Department conferences, rehabilitation inservices, and quality assurance activities
9. Ensures safety of patients, visitors, colleagues, and self
