The RN Case Manager is responsible for the overall direction and supervision of RN's, LPNs, HHAs and ancillary staff in the delivery of direct clinical services to designated Sibley Nursing patients.  This responsibility includes participation in the recruitment, training and orientation of new staff in the performance of their duties.  This position is responsible for ensuring that quality patient care is delivered in an effective, efficient manner using excellent assessment and patient care skills.

 

 

 



Job Requirements

  • Current NYS RN license, Bachelors of Science in Nursing preferred.
  • Must have a minimum of two (2) year’s supervisory experience in Home Care. 
  • Must have knowledge of licensed agency regulations. 
  • Must possess the ability to transmit information clearly and concisely through written and  verbal communications. 
  • Must possess strong organizational skills; with ability to work efficiently under stress, against deadlines with self-direction. 
  • Must possess the ability to develop and maintain good interpersonal working relationships. 
  • Must be willing to be flexible and to meet the needs of the department. 
  • Must have the ability to perform duties days, evenings, nights and weekends, as needed. 
  • Must successfully complete a pre-employment physical and annual health assessments as required by NYS Department of Health.
  • Must have the ability to travel.

 

Physical Requirements

  • Able to use keyboard at a functional level.
