Description
Occupational Therapist/Physical Therapist
The OT/PT is responsible for continuous monitoring and improvement of care that is provided to patients to improve their patients ability to perform tasks in their daily living and work environments in a home care setting. Knowledge of requirements for the provision of home health care including compliance with federal, state and local regulatory agencies. Ability to assess the patient’s needs through the use of the Comprehensive Assessment, including collecting OASIS data at the designated time points required by law; evaluate client's functional status, i.e., muscle function, endurance, visual coordination, written and verbal communications skills, self care ability, work capacity, etc. Evaluate home environment for hazards or barriers to more independent living. Identify equipment needs. Participate in the development and coordination of the total plan of care.

Requirements
· Masters Degree from an accredited program
· Minimum two years experience, one year Home Care experience
· Current and unrestricted Licensure from the State of Michigan
· Current in CPR
· Excellent organizational and communication skills
· Effective listening and critical thinking skills
· Reliable transportation, current driver's license, and auto insurance
