Full time RN Case Manager roles for Union/Somerset/Middlesex counties, Monmouth County, and Bergen County (Wayne, Oakland, and Ridgewood).
Description
RN Case Managers needed for Day Shift roles in either Union/Somerset/Middlesex county, Monmouth county, and Bergen County (Wayne, Oakland, and Ridgewood). In this role you will work with patients in their home or at long term care facilities (roughly 10 patients per week), and manage 2-3 Home Health Aids. Many patients are located in the same facility, so travel time is limited. Must have strong clinical skills, end of life care experience and the desire to work closely with the interdisciplinary team to ensure patient comfort and family support. Very limited, secondary on-call (possibly once per month if that often).
HOURS: 8AM- 4:30PM Monday- Friday. Salary commensurate with experience.

- State of NJ RN License
- 2 years of clinical experience
- 2 years of Hospice experience (preferred)
- Ability to handle high level of autonomy
- Dedication to hospice philosophy and quality patient care
