Visiting Nurse & Health Services of Connecticut (VNHSC) is a not-for-profit organization dedicated to home health, hospice and community-based health care. VNHSC is the parent organization of A Caring Hand whose staff provides private care services designed to help the frail elderly, terminally ill and disabled remain in the comfort of their own homes.

In 2007 we made over 145,000 home visits to more than 17,000 patients, our Hospice staff provided over 15,900 days of care. The organizations Adult Day Center provided 6,900 days of support to the disabled and cognitively impaired in the community.

VNHSC is licensed by the State of Connecticut Department of Public Health and accredited by the Community Health Accreditation Program (CHAP) of the National League for Nursing. VNHSC was named to the 2007 Homecare Elite, a listing of Medicare-certified agencies whose achievements in quality improvement and financial performance are among the top 25% of providers nationwide.

Job Description

                                                                Registered Nurse
                                                         Patient Care Coordinator
*Seeking a Patient Care Coordinator for ENFIELD OFFICE and one for our VERNON OFFICE*
Full-Time


Seeking a Registered Nurse  whom assumes appropriate problem solving approaches to plan performance of services.  Candidate to review and approve all plans of care within agency time-frames.  Facilitates services with consideration of the physical, psychosocial, cultural/spiritual, and age specific needs of those we service.  Responsible for promoting cooperative cohesive group process dedicated to provision of quality of patient care.



Job Requirements

Candidate must possess a current CT RN license from an accredited school of nursing.  Bachelor's degree preferred.  Three to five years of Home Health experience is required.  Case management experience is required.  Excellent working knowledge of OASIS and related outcomes and PPS required.  OASIS certification is preferred.  Individual must have strong analytical and critical thinking skills, detailed orientated and able to prioritize multiple tasks.  Candidate must have excellent oral and written communication skills.  Primarily an office environment, may be required to perform home visits as part of plan of care planning and education process.
