Job Description 

OSF Home Care Services has an opening for a Registered Nurse for Home Health in our Bloomington, IL region.  This position is primarily for 2nd shift and could be part-time or full-time hours.

The RN reviews physicians' referrals and patient's medical records to determine type of home care services needed.  The RN follows physicians' plan of treatment.  Administers prescribed medications as ordered by physicians and monitors patient's response to medication.  Assesses for pain and utilizes pain management.  Reports patient status to physician.  Supervises care given by aides.

The RN is responsible for a scope of care that encompasses all ages from infancy through geriatric.  Assess patient's physical, functional, mental and emotional needs on an ongoing basis, their need for assistance in activities of daily living and coordinates the plan of care with physician, patient, family and other members of the healthcare team.

 


Job Requirements 

  • Currently licensed as a Registered Nurse in the state(s) in which they are practicing.
  • Provides own transportation and auto insurance and abides by all laws, rules and recommendations for safe driving while on duty.  Maintains a valid driver's license.

  • Must have certification in CPR prior to patient contact.
  • One-year recent nursing experience required; home care experience preferred.
  • Expected to take night and weekend calls, and make home visits to patients during the evening, night, weekend and/or holiday hours as required.

  • Responsible for taking on-call on a rotating basis.
  • Must be able to use and operate medical equipment, including Point of Care devices.

  • Demonstrates ability to work independently and execute own workload.
  • Demonstrates critical thinking skills.
