Description

Eyerly Ball Community Mental Health Center is currently seeking a Mental Health Nurse to join our Program for Assertive Community Treatment(PACT).

 

This position will serve as a member of the PACT multidisciplinary team to assess, plan, develop, coordinate and provide treatment, rehabilitation, and support services to program consumers with severe and persistent mental illness under the clinical supervision of the Team Leader and psychiatrist.  Job responsibilities include:

 

1.         Take the lead role or participate in providing medication administration and medical services.

 

2.         Provide case management for an assigned group of consumers.  Ensuring no lapse in services (i.e. Medicaid/Medicare, food stamps, housing, transportation, chore services, home health, etc.).  

 

3.         Assume primary responsibility for developing, writing, implementing, evaluating and revising treatment goals and plans for an assigned group of consumers, within PACT policies and procedures.  

 

4.         Conduct comprehensive assessment of psychiatric history, mental status and diagnosis, physical and dental health, use of drugs and alcohol, education and employment, social development and functioning, activities of daily living and family structure and relationships.

 

5.         Responsibly perform shift management duties according to established policies and procedures.

 

6.         Provide on-call crisis intervention covering nighttime hours and serve as backup to evening and weekend staff.

 

7.         Act as a liaison and consult with community agencies and families to maintain coordination in the treatment process.

 

 

 

 

Requirements

 

1.         Preferred post‑secondary school education and training, including internships and other supervised practical experiences in a clinical or rehabilitation setting with persons with severe and persistent mental illnesses.

 

2.         Minimum education requirements:  BA or BS degree in related field with supplemental license, as applies 

 

3.         Prior work related experience with population served, preferably in a psychiatric team environment.

  

4.         Basic computer skills required
