JOB TITLE: Registered Nurse
The Clinical Nurse provides general nursing care to patients. Assist staff physicians in delivering patient care. Explain procedures and treatments to patients to gain cooperation, understanding and allay apprehension. Aid physician during examinations and treatments, as requested. Observe patient and record significant reactions/conditions. Notify staff physician of reaction to drugs, treatments. Confer with providers; Assist with written prescriptions, refills, phone-ins to pharmacies, and documentation on medication list in chart. Triage daily patient related telephone calls. Assign priority rating and refer to staff physicians as needed. When appropriate resolve patient problem by communicating care instructions and/or advice. Administer treatments, injections, immunizations and intravenous infusions; obtains central venous blood samples, if trained to do so. Patient training on self injection, new medications, or other treatments or issues related to care. As needed, perform duties of Medical Office Assistant taking vitals and updating patient information, collecting samples from patients such as blood and urine, prepare samples for transport and processing by outside laboratories. Perform tests such as EKG as required; accurately record information in patient files. Part-time Monday to Thursday 9:00am to 3:00pm. $25/hr.
Qualifications: EDUCATION and/or EXPERIENCE: Must possess valid NYS license. Previous experience working in a hospital or clinic setting required. Knowledge of HIV/AIDS extremely helpful. Excellent written/oral communication and computer skills are required.
RN Home Care Coordinator
Our client in Rochester is currently recruiting for a RN for their Home Care Coordinator opportunity. The HCC is responsible for the coordination of clinical services between home care and the hospital acute care environment, when the patient’s care is being transferred to home setting. This RN will act as home care resource to hospital or agency personnel regarding care, treatment and services that can be provided in the home setting. Evaluates the needs and care resources for each patient referred to the agency for home care services, including an assessment of the suitability of the patient’s home environment for safely providing care. Obtain the referrals from the physician orders for home services prior to each patient’s admission to home care. Coordinates the home care plan, assuring that services are available as ordered by physician. Document patient’s home care plan and coordination activities. Facilitates communication between assigned hospital/agency, identifying and reporting processes in the system where communication and/or continuity of care can be improved. Qualifications: Graduate of an accredited school of registered professional nursing and valid NYS registered nursing license required; BSN preferred. Minimum of 2 year’s experience as a staff nurse in an home health agency; or equivalent thereof. Fluency and high comfort level on person computers required for computer documentation purposes. Must have ability to travel. Direct Hire, Full-time, 8am-5pm, Monday to Friday.
