Position Summary: HMG Services, L.L.C, a leader in the Skilled Nursing / Long Term Care industry is currently seeking a qualified Nurse Liaison - RN / LVN to join our team of energetic, dedicated professionals. The primary purpose of this position is to represent assigned facilities and company in the Medical Center.
Job Responsibilities / Position Information for the Nurse Liaison - RN / LVN include:
- To conduct and coordinate the development and completion of the resident clinical assessment in accordance with the requirements of assigned facilities and the policies of this Company.
- Act as a liaison between families and the facility during the admissions process.
- To establish an ongoing viable network of referral sources that supports the facility/company and ensures the census and program goals of the facility/company are achieved and maintained.

- Long-term Care knowledge preferred.
- Strong clinical background with emphasis on assessment techniques and evaluation of patients for placement in a skilled nursing/rehab environment.
- Demonstrated success in achieving assigned goals and knowledge of marketing and sales concepts.
- Highly motivated and creative individual with an ability to work independently.
