Harbor Light Hospice is currently seeking an
Assistant Patient Care Manager in Peoria, IL
Harbor Light Hospice uses a team approach to provide quality hospice services to patients and families throughout our service areas.

The Assistant Care Manager participates as a member of the Interdisciplinary group actively helping with coordinating all care aspects of the hospice patient. This involves helping with developing and evaluating plan of care, medications management, and employee education.
The Assistant Patient Care Manager must be able to step into control with proper decision making in the absence of the Patient Care Manager.
Requirements

Candidates must be a Registered Nurse in good standing in the state of Illinois.

Candidates must also have two years of professional personnel management experience.

Hospice experience or management in long-term care setting desired.
