JOB DESCRIPTION:


JOB REQUIREMENTS:

Under general medical direction provides services to individuals with speech, language, and/or hearing disorders by evaluating and establishing short and long term goals based on the individuals current level of speech and hearing related to their disorder. Provides rehabilitative services to direct the patient towards independence and improved mental and physical functioning. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Assists the physician in determining and recommending appropriate speech and hearing services by: a. evaluating and establishing goals based on the patient's current level of speech and hearing related to their disorder; b. reporting to the physician the patient's condition and by; c. developing a plan of care based on diagnostic and functional evaluations to obtain short and long term goals. Develops a plan of care by: a. assessing and documenting the patient's condition on an initial evaluation form according to the patient's diagnosis; b. preparing a patient problem list; c. setting realistic, short and long term goals; d. establishing a plan for frequency and duration of treatments; e. developing a plan for discharge and by; f. documenting and submitting a progress note for each patient visit to office personnel no more than five days from the date care is provided. Directs patient toward independence in physical and mental functioning by instructing and guiding the patient in prescribed therapeutic self-care and creative activities. Participates in the development and revision of the Physician's Plan of Treatment and Plan of Care by recording and reporting to the physician the patient's reaction to the therapy program and/or any changes in the patient's condition. Submits a tally of patient visits made each day by completing a daily activity report and forwarding report to office personnel on a weekly basis. Contract therapists will complete a monthly attendance record and invoice and forward these to billing personnel within one day following the end of each month.
