East Valley Foot and Ankle Specialists provides foot care service to residents in skilled living, aid assisted and independent living nursing homes. We believe in every resident’s right to high-quality healthcare. That’s why we have built a business committed to resident care. We are looking for excellent Nurse Practitioners to join our team in the Phoenix, AZ area!

 

Full or Part-Time Nurse Practitioner

 

Under the supervision of the Clinical Director, the Nurse Practitioner will provide foot services to the geriatric and/or long-term care resident within a long-term care, assisted living or independent living facility. The Nurse Practitioner will adhere to all state-approved medical and nursing guidelines.

 

Responsibilities:

  • Perform assessment of residents

  • Elicit records and interpret the past medical history family and social history as it applies to the resident

  • Order/perform necessary diagnostic tests, procedures and therapeutic interventions

  • Revise the impressions and plan of care based upon subsequent physical exams, laboratory, radiographic findings and overall medical course

  • Record resident data in the medical record or approved database reflecting any changes in the resident’s condition

  • Communicate information regarding the resident’s condition to nursing staff, attending physician, resident and family

  • Collaborate with other healthcare team members regarding the provision of care to the resident

  • Recognize medical problems and complications that require further physician evaluation and treatment and refer appropriately to the attending physician and/or specialist

  • Assist Director of Nursing (DON) or appointed nursing home liaison with education and program training to nursing home staff

  • Provide leadership, consultation and education to nursing personnel and other healthcare providers regarding continence and foot care

 

Requirements

The ideal candidate will have appropriate registrations/approvals to practice as an APN or FPN. Prior nursing home experience is desired. Basic computer skills are a necessity. Valid driver’s license is required. Reliable transportation and car insurance (enforce) is required. Must have the ability to travel up to one to two hours from home for work or training assignments in the Phoenix, AZ area. Occasional out-of-state travel is required for company meetings, continuing education opportunities and/or training. Must be available to work 8:15 AM to 3:30 PM or until finished. Must be flexible with nursing home and be able to drive distances to assigned homes.
