Job Description 
FULL-TIME ON-CALL RN(Weeknights from 4pm-8am)
This position will cover all of our territories in Somerset, Morris, Warren, Hunterdon, Sussex, Passaic and Union counties.
YOU ARE PAID IF YOU GET CALLED OR NOT!!!!
  • Case management of hospice patients in long term care and home settings.
  • Management of nurses aides for your team.
  • Development of individualized plans of care for patients within 48 hours of admission.
  • Provision of direct care, education and support to patients and family members.
  • Management of medical regime as prescribed by attending physicians.
 
Job Requirements  
  • Current and valid RN License
  • 1 year administrative or supervisory experience
  • experience in either Hospice, home health, primary care clinic or other health facility
  • Strong communication skills
  • Reliable transportation
