Occupational Therapist
The Masonicare Health Center, an affiliate of Masonicare, has been in operation for over a century. Masonicare is the largest not-for-profit senior focused healthcare provider in the State of Connecticut. The Masonicare Health Center is located in Wallingford, central to the state and easily accessible off major highways.
We currently have a need for an Occupational Therapist to work Full Time or Part Time (40/32/24 hrs) on the day shift. All are Benefit Eligible Positions!!!
The Occupational Therapist performs diagnostic assessments, plans and implements specific treatment programs and establishes appropriate discharge plans for individual resident/patient participation in selected tasks to restore, reinforce and enhance performance; facilitates the learning of those skills and functions essential for adaptation and productivity. Understands the mission of the organization and the department by recognizing the elderly we serve as unique persons requiring individual alternatives to maintain full lives in their later years by providing medical, spiritual and emotional care, encompassing quality, dignity, compassion and confidentiality for each individual.
Essential Duties and Responsibilities:
1. Assess assigned residents/patients by evaluating data from a variety of sources and incorporating normative findings for older persons. Utilizes appropriate tests and measures. Identifies therapy diagnosis and prognosis. Establishes realistic and measurable short and long term goals considering level of care and discharge destination. Able to assess residents/patients with a variety of diagnoses and in multiple practice settings. Adapts handling skills to meet the needs and abilities of older persons.
2. Establishes realistic individualized treatment programs for residents/patients with a variety of diagnoses and in multiple practice settings. Treatment plan is comprehensive and is geared toward improving functional outcomes. Treatment plan is appropriate to meet the needs and abilities of older persons. Adapts handling skills to meet the needs and abilities of older persons.
3. Demonstrates adequate skill in discharge planning including timeliness of discharge of services or progression to the next level in the continuum of care. Ensures post-hospitalization/treatment needs of the resident/patient are identified and arranged prior to discharge. Establishes and regularly upgrades home programs.
4. Prepares legible, concise and organized written documentation (assessments and reassessments, treatment plans, progress notes, plan of care) that meets external regulatory requirements and departmental guidelines. Written documentation supports billing. Is proficient in use of computer as it related to documentation , billing, e-mail and medical record review and any other areas related to patient care or regulatory requirements.
