Description
Responsible for assisting patients with suspicious clinical findings or cancer symptoms, their families and providers through the continuum of service offerings to resolution of care. Ensures the availability, access and timeliness of elective and urgent services in order to optimize quality of care, coordination and patient experience for breast and other cancer patients.
Edu:
Graduate of an accredited school of nursing. Bachelor of Science in Nursing Required. Master''s degree in nursing or other health-related field preferred.
Edu:
Breast Health Awareness Training Program: Cancer Prevention & Detection Breast Module and Lung Health Awareness Training Program module completion required within six months of hire.
Exper:
Minimum of five (5) years of progressively more responsible clinical nursing experience with health care coordination and/or leadership background.
Exper:
Minimum of three (3) years in general oncology and chemotherapy with breast cancer and/or lung cancer emphasis preferred.
Exper:
Patient Education Experience including in-depth knowledge of current nursing (oncology) theory.
Lic/Cert:
Current license to practice as a registered nurse in the State of Michigan. Clinical Nurse Specialist (CNS) preferred.
Lic/Cert:
Nurse Navigator certificate required within one (1) year from date of hire.
Lic/Cert:
Continuing education required with national (ONS) certification preferred.
Lic/Cert:
Current valid Michigan Driver''s license.
Skill:
Advanced interpersonal skills necessary to interact effectively with patients and their families, physicians, other hospital associates as welll as external agencies or groups.

Ability to communicate effectively both orally and in writing in order to convey goals, processes and issues that may arise and to remain consistent with the SJMO mission and values.

Demonstrated knowledge and skills necessary to provide age-appropriate care in the assigned area. Ability to assess patients'' age-related needs in order to provide optimal care and treatment options.
