Located in Springfield, MA, the Baystate Brightwood Health Center /Cenrto de Salud has an immediate opportunity for a full-time Social Worker with a passion for Community Health.
As a member of a multidisciplinary team of dedicated professionals focused on quality care, unity of family and teamwork, the Community Health Social Worker is responsible for the provision of quality clinical social work services to patients and/or families in an individual or group setting. The successful candidate will have demonstrated knowledge of human behavior and theory; and about the psychosocial impact of disease on individuals and families as it relates to their age and stage of development.
Additional responsibilities include collaboration with other members of our health care team to ensure patients/caregivers have support systems in place that include transportation, food, and housing. Additional responsibilities include: developing collaborative relationships with community resources. Participation in multidisciplinary teams and quality initiatives. Provide minimal counseling, only on an interim basis until community linkages are in place. Must have clear written and verbal communication skills. The social worker must understand they are the first-line for patients in distress.
Bilingual (Spanish) fluent is required. Requires a Masters degree in Social Work. (no other degree is accepted) Massachusetts LCSW required, LICSW preferred. A minimum of two years experience in medical or psychiatric social work with preference to experience in medical social work. Experience within a community health center or agency is highly desirable.
