RN/PATIENT CARE COORDINATOR


REPORTS TO: Administrator

SUPERVISES: This is a non-supervisory position.

RN/PATIENT CARE COORDINATOR Summary:

Serves as a liaison between the Agency and the medical community to coordinate patient care. Ensures that patient care activities are appropriate, timely, and cost effective.

Essential Duties and Responsibilities of PATIENT CARE COORDINATOR Include the Following:


  • Acts as a liaison between the field staff and physicians in obtaining plans of care and appropriate modified orders.
  • Receives information from field staff regarding patient care issues. Prioritizes and reports these issues to the appropriate parties.
  • Reviews findings with the admitting nurse for appropriateness of requested services.
  • Participates in start of care conferences.
  • Recognizes problem areas and facilitates group problem solving.
  • Coordinates care with all rehab personnel (PT, OT, ST, MSW) and obtains appropriate orders for care.
  • Monitors the results of patient laboratory results, procedures, etc., as needed.
  • Ensures that physician’s orders are signed in a timely manner.
  • Attends regularly scheduled meetings with physicians and/or appropriate designees to review patient care issues, orders, 485’s, 60 day summaries, etc.
  • Participates in and conducts joint projects and presentations with other community resources to promote community awareness.
  • Attends agency staff meetings and agency and/or non-agency continuing education programs.
  • Participates in the agency’s quality/process improvement program.
  • Completes home nursing care visits as needed.
  • Contributes to team effort by accomplishing related results as needed.

QUALIFICATIONS for RN/PATIENT CARE COORDINATOR

EDUCATION/EXPERIENCE:
  • Degree in nursing from a program accredited by the National League of Nursing.
  • Minimum of one year nursing experience in an acute care setting preferred.
  • One year home care experience preferred.
  • Demonstrated knowledge and skills necessary to provide care and to communicated with primarily the geriatric population.
  • Demonstrated knowledge of the principles of growth and development over the life span.
  • Able to assess data reflecting the client’s status and report the appropriate information relative to the client’s age specific needs.
  • CERTIFICATES, LICENSES, REGISTRATIONS - Licensed to practice professional nursing or practical nursing in the state. Valid Driver’s license.

  • LANGUAGE SKILLS - Ability to read and interpret documents. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.

  • MATHEMATICAL SKILLS - Ability to add, subtract, multiply and divide in all units of measure using whole numbers, common fractions, and decimals. Ability to apply concepts of basic algebra.

  • REASONING ABILITY - Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to interpret and deal with problems involving several concrete variables in standardized situations.

  • DEGREE OF TRAVEL - Occasional Home visits, weekly office meetings, and frequent travel while promoting community awareness. Must have reliable transportation and agency-required liability insurance.

  • DEGREE OF DISRUPTION TO ROUTINE, OVERTIME - Moderate disruption to routine. Occasional after hours meetings.

  • SAFETY HAZARD IN JOB - Possible infection from clients, auto accidents, unsafe neighborhoods.
