Description
Scope of Position:
Complete telephonic case management assessments and develop a recommended plan of care for policyholders.
Primary Responsibilities:
- Communicate with policyholders regarding medical conditions, care needs and care providers.
- Assist policyholder or family in obtaining care providers as needed.
- Implement ongoing care management functions for active cases.
- Document case management activities and maintain accurate census of policyholders.
- Assist claims examiners in gathering or analyzing medical information.
Secondary Responsibilities:
- Other duties as assigned by Manager.
Requirements
Education: Graduate of an accredited school of nursing leading to diploma,
Associate or Bachelor’s degree in nursing.
Experience: Facilities experience preferred.
Certificates: Current Pennsylvania License for Professional Registered Nurse.
Special Requirements: PC literate; ability to make decisions in fast paced setting; strong communication and interpersonal skills; ability to organize multiple priorities.
