Description
RN Case Manager Job Duties:
- In conjunction with the Interdisciplinary Team (IDT), manage and coordinate the Individual and family/caregiver Plan of Care from referral to discharge.
- Facilitate communication about changes in the Individual’s status among IDT members and the Individual’s attending physician.
- Apply the policies and procedures of Allegiance and the rules and regulations of Federal and State regulatory agencies and other certifying agencies in providing hospice care.
- Plan, coordinate, and deliver direct nursing care to Individuals and families according to the Individual’s need.
- Visit each Individual at least every 14 days in accordance with regulatory requirements.
- Perform a comprehensive assessment of the Individual/family and develops a Plan of Care and ensures the care plan is up to date and reviewed at every team meeting.
Requirements

Current license as a Registered Nurse (RN) in good standing with the State Board.

Must have at least 2 years nursing experience; hospice experience preferred.

Must have demonstrable competency with computers (MS Word, MS Excel, etc.)

Candidates must successfully complete criminal background check and physical(including drug test & TB screening) prior to begining orientation.
