Job Description 

Director of Case Management

Memorial Hermann -Texas Medical Center Campus has developed a highly skilled team of health professionals by attracting some of the best and brightest in their fields.  Together with our partners at UT Medical School at Houston, we have made breakthroughs over the years that have translated to less-invasive procedure, quicker recovery times, improved outcomes for patients, reduced medical coosts and increased employee and patients satisfaction.  Our team is continuing to build on the hospital's long legacy of accomplishments.  And this is just the beginning ....join our team of leaders.

The Director of Care Management is responsible and accountable to work with the Directors of Case Management on the implementation of the case management program at the local level.  The components/roles of the inpatient case management program consist of the following: Care Facilitation, Utilization Management, Case Management and Discharge Planning.
 


Job Requirements 

Director of Case Management
1.  Master's degree required
2.  At least 10 years experience in utilization management, case management, discharge planning or other cost/quality management program.
3.  At least 5 years experience in hospital-based nursing.
4.  Knowledge of leading practice in clinical care and payor requirements.
5.  Self-motivated, proven communication skills, assertive.
6.  Background in business planning, and targeted outcomes.
7.  Working knowledge of managed care, inpatient, outpatient, and the home health continuum, as well as utilization management and case management.
8.  Working knowledge of the concepts associated with Performance Improvement.
9.  Demonstrated effective working relationship with physicians.
