Job Description 

 

Working in the NewCourtland LIFE program, you’ll have both the autonomy to apply your clinical skills and the support of an interdisciplinary team. As part of the award-winning NewCourtland Network, the new Living Independently For Elders (LIFE) program is an innovative, community-based program that supports elders living in their homes. Under the direct supervision of the Medical Director, the Nurse Practitioner of the LIFE Program will apply the advanced knowledge and clinical skills necessary to assess and manage the healthcare of chronically ill geriatric participants.  In addition, you will provide clinical skills development training to the nursing staff and assist in related administrative and program development tasks.

 

 


Job Requirements 

 

The qualified candidate must be a graduate of an accredited Nurse Practitioner Program with at least 3 years experience in adult or geriatric primary care, and have licensure as a Nurse Practitioner in Pennsylvania and Certification in their specialty area by ANCC as well as AANP.  The potential candidate should have at least 3 years of prescribing experience. In addition, qualified candidates must possess a Bachelor’s Degree in Nursing from an accredited college or university and have current RN licensure in Pennsylvania, a valid PA Driver’s License, and proven experience providing care to an older adult/elderly population is required.  MSN in Gerontology or Adult specialties by an accredited Nurse Practitioner Program are preferred. Candidate must be eligible to apply or have current DEA certification.
