Description

The practitioner/supervisor will provide family planning services 2-3 days per week to women, men and teens in family shelters. He/she may provide well-child care if appropriately certified. The practitioner/ supervisor will also supervise 3 community health nurses and a part-time nurse practitioner providing family health services in family shelters. The practitioner/supervisor will collaborate with the clinical director and infection control coordinate on service planning and oversight.  He/she will also collaborate with shelter directors and leaders of other shelter-based service organizations. The practitioner/supervisor will report to the Health Care for the Homeless Clinical Director.

 

RESPONSIBILITIES:

Clinical

·         Provide comprehensive family planning services and well-child care (if certified)

·         Assist with NP or community health nurse coverage as needed

·         Participate in outbreak management under the guidance of the infection control coordinator

·         Document all services in a timely manner

Supervision and management

·         Participate in management team with Infection Control Coordinator and Clinical Director on policy, plans and service coordination.

·         Orient and supervise staff providing outcomes-oriented family planning, well-child and other health services in shelters

·         Develop and maintain on-site HCH teams designed to achieve clinical outcomes and to maximize effectiveness of limited NP services

·         Oversee tracking of follow-up, HIPAA compliance, medication safety, and other risk-management activities.

·         Routinely monitor and report outcomes based on HCH data, on-site logs, and chart review.

·         Develop collaborative relationships with community health care providers

·         Notify immediate supervisor of any significant problems including client care and changes/issues at a site, and system problems

·         Notify Infection Control Coordinator immediately of suspected or confirmed communicable diseases which can be transmitted through routine shelter contact

·         Advocate for consumers and HCH program, as appropriate

·         Participate in advocacy and coordination committees addressing issues related to homelessness

·         Assume new responsibilities as new needs are identified.

Requirements

SKILLS

Good written and verbal communication skills in English

  

EXPERIENCE:

·         5 years in nursing or related field including 1 year as nurse practitioner

·         Experience in community nursing

·         Supervisory experience preferred.

 

EDUCATION REQUIREMENT

·         MSN or equivalent

·         Pennsylvania CRNP – Family or Adult health
