Description Patient Care Resource Coordinator 
 
The Patient Care Resource Coordinator position is designed to assist OCHS patients and their families in navigating and linking to all available community resources across multiple sites.  This position will work in collaboration with the OCHS medical, dental and behavioral health providers in developing connections to resources that will supplement and enhance the overall treatment and well-being of the patients we serve.

Position is full time, Monday - Friday 8:30 to 5:30.  Full benefits are provided, such as health and dental insurance, Short/Long term disability, 403b Retirement and Life Insurance, as well as Holidays and accrued Vacation.

Requirements
  1. Bachelor's degree preferred.
  2. Bi-lingual English/Spanish and computer skills preferred.
  3. Previous Case Management experience a plus.  
  4. Positive work attitude with an ability to work as a team with other professionals and to carry out responsibilities with minimal supervision.  
  5. A committment to continuous, quality improvement. 
  6. Flexibility (ability to adapt to changing environment).
