Job Description 

Hospice Registered Nurse / RN Case Manager

 

Midwest Palliative & Hospice CareCenter is a non-profit community based healthcare organization promoting comfort and quality of life for patients with life-threatening illnesses. Our services address physical, emotional, interpersonal and spiritual concerns.

 

We are looking for a RN / Registered Nurse.  Current opennings for full and part time in our Lake County region.

 

Primary duties to include:

  • Provide home/facility-based skilled nursing to hospice patients.
  • Provide support to families and caregivers.
  • Develop an interdisciplinary and holistic plan of care for each patient and family.
  • We have  full-time and part-time positions serving the near north and northwest suburbs.

Job Requirements 

Hospice Nurse /  RN Case Manager

  • 1 year nursing experience
  • RN license in Illinois
  • Driver’s license
