Nurse Liaison / Admissions Marketing Nurse - Denver area

Sava Senior Care is a premier provider of long term care skilled nursing services. Sava is a national company. We have affiliated facilities throughout the state of Colorado. Our facilities are built around respect for it's residents and it's employees.

We currently have an opening for a Nurse Liaison to assist with Admissions and marketing to referral sources in the Denver Metro area hospitals.

A Nurse Liaison does the following:
SUMMARY

Establishes and maintains positive relationships with referral sources. Conducts physical assessments and/or screening of potential residents.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
· Represents the company with professionalism to referral sources (i.e., care coordinators, discharge planners, social workers and physicians). Develops and maintains positive relationships to gain referrals.
· Assesses potential residents/patients to ensure facility is able to provide appropriate level of clinical care.
· Converts at least four (4) referrals to admissions per day.
· Travels to hospitals to assess acute care residents; may also be required to do home-site visits.
· Serves as a member of the sales team; presents cases on a weekly basis to department managers to determine facility needs.
· Meets with potential residents/patients and families to educate them of the facility services offered.
· Maintains contact and coordinates care with patients who return to hospital from facility to ensure facility care was satisfactory.
· Coordinates pre-authorization for managed care admissions.
· Completes required forms and documents in accordance with company policy and state and/or federal regulations.
· Maintains a strong working relationship with attending physicians in area hospitals to promote skilled services, assist with knowledge of geriatric medications and assist with drug interchanges.

 

Job Requirements

· Registered Nurse preferred.
· RN must have a minimum of one (1) year experience in patient/resident assessments;
· Maintains an appropriate level of knowledge about sources of payment.
· Must be able to travel 75% of the time on day trips to multiple hospitals in the area.
· Must have excellent communication and presentation skills; have skills to conduct basic sales presentations.
· Proficient in Microsoft Office products (Word, Excel, PowerPoint).
· Must understand the admissions process from acute care to long-term care, skilled nursing.
