Director of Case Management – Centennial Medical Center – Registered Nurse

Come be a part of a facility that continues to raise the bar for healthcare on both a regional and national level, serving Nashville, middle Tennessee and the southeastern United States.  Our organization is currently seeking a well-qualified Director of Case Management to oversee a department within one of the largest, full service, hospital settings in Middle Tennessee.

The Director of Case Management directs, monitors, and evaluates the quality and effectiveness of case management, social services, discharge planning, and utilization review activities. The tasks and responsibilities include but are not limited to:

• Provides leadership and direction to the department.
• Develops and implements strategies to facilitate timely and appropriate admissions.
• Assesses and identifies needs of the program, continually improves the process and functions related to case management.
• Monitors and facilitates proper utilization of resources.
• Facilitates overall UM program within the Revenue Cycle, including Physician Utilization in conjunction with the Physician Advisor/Chief of Staff.
• Measures, analyzes, and reports outcome data internal/external including length of stay and case mix index to keep the facility within expected ranges.
• Oversees performance improvement programs including patient satisfaction and patient/family involvement in the discharge planning process.
• Interfaces with Managed Care contracting
• Communicates with Division Case Management Leadership.
• Participates in Case Management division initiatives.

Successful candidates will have:
Current RN TN License
BSN, MS preferred

Three or more years of Director / Manager level experience in acute care Case Management.
