Its services include intake coordination, benefits and reimbursement support, medication fulfillment and administration, compliance and patient education, one-on-one care and customer service. Skilled nurses and pharmacists are accessible 24/7 to deliver and manage complex infusion and injectable therapies from more than 100 OptionCare home infusion and specialty pharmacies in 35 states. OptionCare's services also include respiratory therapy and home medical equipment at some locations.



Job Duties Include:  


Plans, coordinates, provides and evaluates home care services to patients. Ensures the clinical soundness of the assessment and proper delivery of services to patients within the Home Care setting. Ensures provision of all covered services identified in the treatment plan. Provides continuous evaluation of the effectiveness of treatment through ongoing assessment of the patient. Interacts with area service providers on a daily basis to conduct assessment of patients selected by the hospital for care and who have been referred to Walgreens-Option Care. Communicates timely with the discharge planner/ social worker to coordinate needed Home Care services on patients that have been referred to Walgreens-Option Care. Effectively communicates with agency staff, medical team, patients, and family. Effectively Communicates with Marketing Staff and Development of Educational and promotional programs. Maintains a professional image. Participates and attends required inservices.



 

Requirements


 

  • Licensed Nurse in the state of practice with 2-3 years of experience in the Home Care setting. 
  • Must posses management  problem solving and organizational skills.
  • Must possess strong clinical skills.


  • Preferred Qualifications:


    Previous Home Health Care and/or previous Infusion experience preferred but not required.

RN, Dietitian or MSW preferred.
