INSPIRIS is a unique care and care management company focused on improving the health care and quality of life for the medically complex, chronically ill, frail and elderly patient through Medicare Advantage and Medicaid health plans. INSPIRIS offers nurse practitioner led programs for Skilled Nursing Facilities, Assisted Living or other congregate housing and in individual residences. These programs can be as short as a single visit for clinical and HCC Risk Scoring assessment, short term for management of post-hospital discharged members to long term for those members not accessing the current PCP network. We are currently seeking Nurse Practitioners for the expansion of our Home Visit program into Detroit and Grand Rapids, MI.
Our Home Visit program delivers high quality medical care to chronically ill, elderly patients in the comfort of their own home. As part of a team of providers and support staff, the Nurse Practitioner (NP) will have enough time to evaluate the patient's needs and develop a proactive and preventive healthcare plan. The NP is provided all the necessary equipment and medical supplies needed to provide care in the home, and utilizes an electronic medical records system (EMR) to accurately and efficiently document their visits. The NP is responsible for establishing and maintaining effective relationships with patients, key decision-makers, and collaborating physicians. Nurse Practitioners with the desire to really make a difference in the lives of the patients will find this practice very rewarding. This position requires travel to patients' homes and shared call participation. Reports to a Network Medical Director.
