Description RN- Telephonic Case Management

Interim Healthcare is currently recruiting for an RN to work on telemonitoring team focusing on comprehensive disease management. Candidates will have outstanding communication skills and always act with the highest principals and are guided by what is ethical and right for their patient. This is a full time assignment with standard weekday hours with no weekends, no holidays and no overtime. Job will be based in the West Metro. Must be familiar with dialysis and end stage renal failure.

Requirements

Current MN RN license
Current CPR
1 yr of dialysis experience
Disease Management/ Case Managment eperience is preferred
