Job Description

Scheduling Coordinator

Bradenton, FL

 

The Coordinator is responsible for maintaining the schedules of RNs, LPNs, and other skilled/non-skilled field personnel to perform tasks and services to meet patients' needs, within the patient's place of residence.

 

Essential Duties and Responsibilities:

 

Schedules and assigns field personnel to cases in a timely manner

Accepts referrals by phone or fax and maitains necessary log

Assists in coordinating services provided to clients

Controls and monitors schedule changes

Checks compliance of visits done after notes are attached to schedule.  Brings any schedule problems to supervisor's attention immediately.

Gives and takes reports from the the on-call coordinator

Attends meetings as requested by supervisor

Obtains weekly visit count and report results to Director

Assists in relaying messages to field staff, office staff and PCCS

Works to develop a positive rapport with all staff members and community resources affiliated with home health care services

Maintains the client, physician, referral source and schedule database

Assists with verifying information for PR and Billing

Participates in on-call process

Job Requirements

Qualifications and Experience:

 

High scholl graduate or equivalent

One (1) year experience as a coordinator or other healthcare experience

Must posses positive enthusiastic attitude, good people skills and professional appearance

Ability to read and interpret documents and procedure manuals

Computer data entry skills

Employee must be able to perform multiplee tasks in a fast paced environment, without constant supervision
