We are currently looking to fill an Regional Care Management Coordinator position for our Gulf Coast Region (Greater Houston Area). The position would cover a total of 8 facilities.
Job Summary:
This position coordinates the education and training of regional and facility employees in the Care Management Process and the use of various supporting software applications to ensure an appropriate level of Resident care as well as equitable and timely reimbursement of the care provided.
Responsibilities
*Provides information and education to facility and regional employees on the importance and use of the RAI / PPS / VA processes and their correlation to resident care and reimbursement.
*Provides instruction on how to read, interpret and use the information contained in various state and company generated reports, such as State Missing Assessment Report, Quality Indicators, Quality Measures, State Validation, etc.
*Based on information derived from audits and other available data, the Coordinator provides expertise, consultation and guidance to appropriate employees to ensure MDS accuracy and adherence to company policy and procedures as well as state and federal regulations and guidelines.
*Through analysis of financial and clinical reports, Coordinator identifies trends and areas of improvement and ensures an appropriate action plan is completed.
*Maintains in-depth knowledge of the clinical software billing system and the MDS process to help investigate / troubleshoot user-related software issues.
Interviews applicants for the Resident Care Management Director position (RCMD) and provides recommendations to the hiring authority.
*Oversees and coordinates the process for a thorough and timely orientation for new RCMDs.
*Ensures current, new, and revised RAI / PPS information is distributed to appropriate employees in a timely manner.
*Provides information to directors, vice presidents, regional staff and corporate on the status of clinical and financial reimbursement.
*Participates in national and state professional organizations that strengthen the company’s position and knowledge of the RAI process, Case Mix and clinical software programs.
*Frequent overnight travel is required.
