Licensed Social Worker - LSW

Ellicott City Nursing and Rehabilitation Center is a proud member of the CommuniCare family of health care providers. Ellicott City is currently recruiting a Licensed Social Worker with long term care experience to join our World Class Team! Care Plan experience and MDS experience preferred.
Successful candidate for the LSW position will assess, plan, coordinate and implement services to enhance our resident's social and psychosocial well being and assure that the highest degree of quality care is provided at all times. The position of Social Worker is a full time position, working 40 hours a week.
Responsibilities of the LSW include but are not limited to:
- resident advocacy
- one on one interactions with those residents who are bed-bound
- ongoing assessments
- assistance with residents' and family members issues and concerns as they arise
- care conferences with resident and their family members.

Requirements
Successful candidate will have:
- A license through the State Social Worker and Counselor Board.
- A Bachelors Degree in Social Work or related field.
- Prior long term care experience a must!
- Care Plan experience and MDS experience a plus.
