Provides education regarding home care to appropriate health care professionals and the community for the purpose of increasing awareness of Baptist Trinity Home Care & Hopsice. Manages and facilitates intake coordination activities necessary to arrange for the uninterrupted continuing care of the patient. Acts as a clinical liaison in the development and coordination of the plan of care and communicates same to all involved parties. Under the general direction of the Director of Community Education.
Licensed RN in the State of TN or compact license.
Minimum of two years experience in home health and one year of hospital experience preferred.
General knowledge and operation of a computer along with other general office equipment.
Ability to communicate effectively with co-workers, patients and their family, and physicians.
